Important information regarding immunization compliance for Glendale campus students.

Immunization & Titers Requirements

When starting as a student, you must be in compliance by submitting all documentation for immunizations and titers from your Immunization Checklist. All immunization documentation is submitted through your student portal.

Step 1: Review the Immunization Checklist

Students must review and provide the appropriate documentation.

A student can request a waiver for a University immunization requirement from their respective College Dean/Program Director and the Dean of Students, but the student must be aware that the requirements are established by affiliated clinical rotation sites as well as ÃØÉ«´«Ã½. Contact Student Services if you have questions at azstudentservices@midwestern.edu.

Step 2: Download & Complete the Required Forms

Download the required form and take to your healthcare provider. Please login to the to find:

  • Student Health Status Report (Physical for new students)

If you are required to have titers, you need to either have your hometown doctor order it or come to campus and establish with one of the healthcare providers at the Multispecialty Clinic or get a lab requisition form from Student Services. If you need a booster, you would either have your hometown doctor order it, come to campus and establish with one of the healthcare providers at the Multispecialty Clinic, or go to a local pharmacy (call the Pharmacy ahead of time). Appointments at the are required for your vaccines, titers and TB testing. To schedule an appointment, please call 623-537-6000.

Health insurance doesn't always cover blood titers so please be aware of this and check before you have them done as they can be costly. If you will have the Aetna student insurance the titers are covered. The expense of the blood titers will also be included in your financial aid package which you will receive the first week of classes.

You are responsible for periodically checking your immunization compliance status by logging into the . If you notice information you believe is incorrect, please contact Student Services.

Important: Some programs require titers in addition to vaccine records. Please see the program specific requirements section for a listing of those programs. 

Immunization

Requirement

Childhood and/or adult vaccine records

  • Proof of (3)-childhood Hepatitis B vaccines
  • Proof of (2)-childhood MMR (Measles, Mumps, Rubella) vaccines (given after your 1st birthday)
  • Proof of (2)-childhood Varicella vaccines

If you do not have vaccine records, you will be required to obtain blood titers for those missing vaccines. A titer is a blood test that measures circulating levels of antibodies.

  • Hepatitis B Surface Antibody titer
  • Varicella IgG titer
  • Measles IgG titer
  • Mumps IgG titer
  • Rubella IgG titer 

Current Tdap vaccine (Tetanus, Diphtheria, and Pertussis)

  • Within the last 10 years
  • Given after age 11

Flu Vaccine

  • This is an annual requirement.
  • Annual flu shots are available starting early August each year.
  • There is no need to upload flu shot documentation from the previous year.

TB Clearance

  • IGRA (Interferon Gamma Release Assay) blood test (Quantiferon-Gold or T-Spot) or (2)-Step TB skin test
  • The IGRA blood test is preferred since it is only 1 visit to the lab for the blood-draw; and a more objective test.
  • A (2)-step TB skin test consists of 2 different TB skin tests completed within 1-3 weeks of each other but not read sooner than 7 days apart. This requires 4 separate visits to the clinic to have both tests placed and read.
  • This is an annual requirement for students in some colleges. Once you have a baseline IGRA blood test or (2)-step TB skin test, you may only need to complete a (1)-step TB skin test each year – provided it is done before the expiration of the prior year’s test. An IGRA test (Quantiferon-Gold or T-Spot) is also acceptable to satisfy the annual requirement.

Positive TB test results

  • If you receive a positive TB skin test, you should obtain an IGRA blood test to rule-out a false positive test. You should never receive another skin test, as you will always test positive. This is especially true if you received BCG vaccine as a child. An IGRA blood test is not affected by previous BCG vaccine.
  • You will need clearance of signs/symptoms of TB from your Healthcare Provider (HCP). They may or may not recommend a Chest Xray (CXR) and/or treatment for latent TB. We do not accept CXR as TB Clearance. Your HCP may write a letter for you to upload to your student portal, or they can complete the Positive TB Questionnaire, which will be emailed to you upon receipt of your positive test result submission.

COVID vaccine(s)

  • Not currently required by the University
  • Initial COVID vaccines series and updated booster are strongly recommended and may be required by some programs and clinical rotations.

  • Rubella IgG titer

Dental Medicine

Baseline TB clearance only at matriculation

Nurse Anesthesia - Entry into Practice

  • Titers required:
    • HepB Surface Antibody
    • Varicella IgG titer
    • Measles IgG titer
    • Mumps IgG titer
    • Rubella IgG titer
  • Immunization records due prior to beginning of 2nd year

Nurse Anesthesia - Online

No immunization requirement

Physician Assistant

Updated COVID vaccine within 12 months of the start of clinical rotations:

  • If vaccinated; 1 updated dose of Comirnaty or Spikevax
  • If unvaccinated; 1 updated dose of Comirnaty or Spikevax; or 2 doses of
    updated Novavax

Podiatry

Titers required:

  • HepB Surface Antibody
  • Varicella IgG titer
  • Measles IgG titer
  • Mumps IgG titer
  • Rubella IgG titer

Veterinary Medicine

  • 1-step TB skin test acceptable for baseline
  • Baseline TB clearance only at matriculation
  • No HepB requirement
  • 2-dose rabies vaccine or positive rabies titer required prior to matriculation. 

ÃØÉ«´«Ã½ has updated our system for submitting immunization documentation. All documentation MUST now be submitted electronically by uploading your documents as PDF files.

Upload Immunization Documentation to the Student Portal

  1. Log in to the ÃØÉ«´«Ã½ Student Portal
  2. Click on the heading for Student Services
  3. Under the General Information tab select Forms & Information
  4. Select Documentation Upload
    1. This will take you to the place where you upload your documentation.
    2. The instructions for uploading your documents are located on this page. If you hover over one of the categories, the information for the category will appear.
    3. Documents must be in a PDF format
    4. Create separate PDF files for each immunization requirement
      1. Flu Shot
      2. Student Health Status Form
      3. TB Testing
      4. Tdap/Tetanus
      5. Titers
      6. Vaccines

Student Services will then enter your immunization documentation into the tracking system. Please allow at least 1 week from the date you upload your documents before calling to ask about your documentation. If there is an issue with any of the documentation you submit you will receive an email explaining what the problem is. Once you have completed all of your requirements you will receive an email telling you that you have completed all of your requirements. 

You will be able to monitor your immunization compliance by doing the following:

  1. Log in to the ÃØÉ«´«Ã½ Student Portal
  2. Click on the heading for Student Services
  3. Under the General Information tab select Immunizations

Once your documentation has been entered the date fields on your Immunization page will be populated. The expiration dates for Tdap/Tetanus and TB Testing (TB Skin Test or TB Quantiferon) will also be listed. All of your uploaded immunization documents will be on your Immunization page as an attachment under the Document Type section. You can open the attachment and print this off for providing documentation as needed. There is no need for you to contact Student Services to get copies of any of your documentation as it will all be available on your immunization page.
